Liability WaiverI acknowledge that participation in soccer involves physical activity and risk of injury. I hereby release Dream Team FC, its coaches, staff and volunteers from any liability for injuries sustained during training or games, except in cases of gross negligence.
Photo & Video Permission (Optional)I grant Dream Team FC permission to photograph and/or video record my child during training, games and events for use on the website, social media and promotional materials. You may opt out at any time by emailing info@dreamteamsoccer.com.
Payment AuthorizationI authorize Dream Team FC to charge my selected payment method for the full season fee. I understand the season commitment cannot be cancelled once started and my spot is secured upon payment.
